Job summary

Community Options, Inc. in Pittsburgh, PA is seeking a Full-Time Community Integration Counselor to assist individuals with learning essential life skills and to help them live more independently in the community.

The CIC provides individualized support, conducts regular visits, documents progress, coordinates transportation and activities, and collaborates with the care network while meeting mandatory background checks and licensing requirements.

Qualifications

• High school diploma or GED; Bachelors degree preferred.
• Valid drivers license with a satisfactory driving record.
• Minimum of two years of relevant experience supporting individuals with intellectual and developmental disabilities.
• Ability to establish a comfortable and supportive relationship with individuals receiving supports.
• Employment contingent upon successful completion of checks (criminal background, central registry, child abuse registry, and drug testing).

Responsibilities

• Support and encourage individuals to increase their independence.
• Meet waiver participants regularly in residence, community, or office settings.
• Provide ongoing support for community integration efforts.
• Document progress toward acquiring community integration.
• Assist in building a support network between the individual and the community.
• Administer medications and ensure proper documentation.
• Coordinate all leisure and volunteer opportunities.
• Monitor health and medical needs and report concerns.
• Drive individuals to medical appointments and community activities.
• Ensure program documentation and billable records are completed accurately and timely.
• Additional tasks may be assigned; cooperate with licensing and department staff during inspections.
