Job summary

Community Options, Inc. in Camp Hill, PA is seeking a Full-Time Community Integration Counselor to help individuals build life skills and independence in the community.

The CIC will work with waiver participants, meeting them in home, community, or office settings, and will document progress toward greater independence. Responsibilities include coordinating leisure activities, transporting to medical appointments, and ensuring accurate program documentation.

Qualifications

• High school diploma or GED (Bachelor degree preferred).
• Valid driver’s license with a satisfactory driving record.
• Minimum of two years supporting individuals with intellectual/developmental disabilities.
• Ability to establish a warm, supportive relationship with participants.
• Criminal background checks and related screenings required.

Responsibilities

• Support individuals to increase independence and daily living skills.
• Meet waiver participants regularly in residences, community, or office settings.
• Provide ongoing community integration support and document progress.
• Assist in building community connections and support networks.
• Administer medications and ensure proper documentation.
• Coordinate leisure and volunteer opportunities; transport to appointments.
• Monitor health needs and report concerns immediately.
• Ensure accurate program documentation and billable records.

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.

We are now hiring a Full-Time Community Integration Counselor in Camp Hill, PA. The Community Integration Counselor (CIC) is responsible for assisting individuals with learning physical, intellectual, emotional, and social life skills. The Community Integration Counselor provides individualized services designed to assist participants in managing and overcoming the difficulties confronted when living in the community.

Starting pay is $17.80/per hour

Responsibilities

• Support and encourage individuals to increase their ability to be as independent as possible
• Meet with waiver participants on a regular basis either in the individual’s residence, the community, or an office setting
• Provide ongoing support for community integration efforts
• Document individual progress toward acquiring community integration
• Assist in building a support network between the individual and the community
• Administer medications and ensure proper completion of necessary documentation
• Coordinate all leisure and volunteer opportunities
• Monitor the health and medical needs of individuals and immediately report any concerns
• Drive individuals to medical appointments and activities in the community
• Ensure program documentation and billable records are completed accurately and timely
• Additional tasks and responsibilities may be assigned
• Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation
  
  

Minimum Requirements

• High school diploma or GED, Bachelor’s degree preferred
• Valid driver’s license with a satisfactory driving record
• Minimum of two years of relevant experience supporting individuals with intellectual and developmental disabilities
• Ability to establish a comfortable and supportive relationship with individuals receiving supports
• Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing
