Job summary

ServiceSource is seeking a Community Integration Specialist in Fairfax to support adults with developmental disabilities in daily programming including academic, social, life and leisure skills.

You will develop and implement Person Centered Plans, promote community inclusion, monitor progress, and maintain PCP documentation while ensuring safety and compliance with Medicaid, Licensure, and CARF standards. Excellent communication and teamwork are essential.

Qualifications

• High School diploma (or GED) required; plus one (1) year experience with adults with developmental disabilities.
• Preferred: college courses in human services or a bachelor's degree with related experience.

Responsibilities

• Provide active engagement and support for participants in community integration, training, leisure, communication and mobility.
• Assist in developing and implementing Person Centered Plans (PCP) and maintain documentation.
• Ensure safety, cleanliness, and regulatory compliance; report incidents per policy.
• Communicate with families, guardians, and coordinators regarding progress and needs.
• Participate in trainings and team meetings; support staff and participants as assigned.
