Job summary

Easterseals seeks a Community Inclusion Specialist/Job Coach for the First Shift in Waterbury, CT. This full-time role delivers hands-on support to individuals with disabilities, focusing on vocational and life skills, within Employment and Community Services.

Under supervision, you’ll participate in interdisciplinary teams, document progress, and help participants achieve Independent employment and daily living goals. Strong communication and organizational skills required.

Qualifications

• Requires 21+ years of age with a valid driver’s license.
• CPR & First Aid certification, PMT, and all required state trainings within the required timeframe.
• Eligible to obtain PSL based upon program needs.
• Eligible to obtain DDS medication certification based upon program needs.
• Clear and effective communication skills.
• Excellent problem-solving skills.
• Self-directed who can prioritize workload.
• Proficient with Microsoft Office Suite.

Responsibilities

• Works directly with individuals in employment settings and/or program models to meet goals.
• Implements individualized treatment plan goals, Behavior Support Plan, health protocols, teaching strategies, and reports.
• Provides on-site employment and life skills training and acts as a community liaison.
• Assists individuals in accessing community activities and services.
• Develops community relationships to expand opportunities for independence.
• Provides transportation as assigned.
• Assists with personal care as prescribed.
• Completes required documentation and trainings.
