Job summary

You will transport participants, support daily activities, and help people participate in their communities, while working in a collaborative team with a 1:2 staff-to-client ratio.

Requirements include being 18+, a valid driver’s license, CPR/First Aid, DOJ clearance (Live Scan), and reliable transportation, with 80% community mobility.

Qualifications

• Must be 18 years of age.
• Possess a valid Driver’s license.
• Move about in the community 80% of the time.
• Must have CPR/First Aid Card.
• DOJ clearance (Live scan) required.
• Reliable transportation required.

Responsibilities

• Willingness to transport participants in a personal vehicle.
• Work at a 1:2 staff to client ratio.
• Promote person-centered thinking, client choice, independence.
• Provide support in recreation, socialization, health and safety, and community inclusion.
• Maintain schedules, records, and documentation.
• Participate in assessment and data collection; report writing.
• Attend required training and staff meetings.
• Other duties as assigned.
