Job summary

Easterseals is hiring a Community Inclusion Specialist to work directly with individuals with disabilities across employment and life-skills programs. You will implement treatment plans, provide on-site training, and collaborate with interdisciplinary teams to empower clients towards greater independence.

The role includes community liaison work, transportation support, and adherence to safety and documentation standards.

Qualifications

• High School Diploma or equivalent required; Associates degree preferred.
• CPR & First Aid certification and all required state trainings within required timeframe.

Responsibilities

• Works directly with individuals in employment settings and program models to meet goals.
• Implements individualized treatment plan goals and Health/Behavior protocols as part of interdisciplinary teams.
• Provides on-site employment and life skills training and acts as a liaison in the community.
• Assists individuals with daily living needs and facilitates access to community activities.
• Develops and expands community relationships to enhance independence.
• Provides transportation as assigned and assists with personal care as prescribed.
• Completes documentation (Therap, binders, emails) and attends required trainings and meetings.
