Job summary

ChristianaCare's Hope Center seeks an autonomous Registered Nurse to join our Community Impact Health Team. You will provide evidence-based nursing care to diverse patient groups in both virtual and onsite settings, coordinating care and advocating for patients to improve outcomes.

This role blends direct patient care with case management, quality improvement, and education of Medical Assistants and Licensed Practical Nurses, supporting a compassionate, patient-centered approach.

Qualifications

• RN license required or eligible to practice in Delaware.
• Two years’ RN experience; physician office experience preferred.
• BLS certification required.

Responsibilities

• Provide evidence-based nursing care to pediatric to geriatric patients in virtual and onsite settings.
• Conduct wellness visits, advance care planning, transitional care management and chronic disease interventions.
• Perform clinical procedures (POCT, EKGs, meds, routine treatments) and patient monitoring.
