Job summary

ChristianaCare Health System's Hope Center seeks an experienced RN to join the Community Impact Health Team. You will navigate patient care across virtual and onsite settings, coordinating with providers and the interdisciplinary team to improve outcomes for vulnerable populations.

The role blends direct patient care, case management, and quality improvement, with leadership and mentoring opportunities and a mission-driven environment.

Qualifications

• RN license or eligibility to practice in Delaware.
• Two years of RN experience; pediatric, adolescent, adult, and geriatric care.
• BLS certification required.
• BSN preferred; ADN accepted with BSN within 3 years.

Responsibilities

• Provide evidence-based nursing care in virtual and onsite settings.
• Conduct wellness visits and chronic disease management interventions.
• Perform procedures including POCT, EKGs, meds, and patient monitoring.
• Triage walk-in patients and respond to urgent concerns.
• Educate patients on wellness, adherence, and self-management.
• Collaborate with physicians and interdisciplinary team to ensure quality care.
• Support quality, safety, and accreditation initiatives.
