About Our Team

Our care model supports members in their homes, communities, and care hubs. As part of our team, you will support patient-facing teams to deliver care with empathy, respect, and non-judgment. Your role is to ensure our teams are fully supported to provide excellent care to our members.

Responsibilities

Supervision and Team Management:

  • Monitor and evaluate team skills, providing ongoing education and training
  • Develop tools to document performance
  • Ensure team members are punctual and fulfill job functions
  • Assess progress on program goals
  • Address blockers/issues and escalate when necessary
  • Supervise the Promotora de Salud (PdS) team through regular check-ins and performance reviews

Documentation and Quality Assurance:

  • Ensure timely, complete documentation of PdS activities
  • Implement quality assurance measures
  • Continuously assess performance against targets

Program & Event Coordination:

  • Participate in changes to program models
  • Support administrative activities for community events

Team Support & Additional Functions:

  • Assist direct reports with assessments, needs identification, and referrals
  • Facilitate case conferences for team support
  • Be available on 'on-call' basis for team consultation
  • Oversee operational data collection and reporting
  • Own issues to ensure positive member experiences and escalate feedback for improvement

Requirements for the Role

  • Bachelor's Degree or equivalent experience in social services
  • 3+ years in healthcare setting
  • Experience with Medicaid/Medicare
  • 2+ years as a CHW/PdS or similar role
  • 2+ years supervising frontline staff
  • Experience working directly with patients, providers, and healthcare teams
  • Care coordination, advocacy, or navigation experience
  • Leadership, decision-making, and calmness in challenging situations
  • Skills in supervision, counseling, teaching, motivating, and team building
  • Proactive learner, adaptable to environments
  • Passion for community work
  • Customer service orientation
  • Excellent communication skills, including listening
  • Strong computer literacy
  • Adherence to HIPAA confidentiality rules
  • Fluent in English & Spanish (verbal and written)
  • Passion for community engagement
