Job summary

Northwell Health seeks a Community Health Worker to act as liaison and support for vulnerable community members, conducting assessments, in-home visits, and referrals to social services to address material and health challenges. You will maintain a caseload, coordinate care with the care team, and help connect members with appropriate services.

You will provide health education, coach on chronic conditions and self-care, and work to empower individuals while building positive relationships

Qualifications

• High School Diploma or equivalent is required.
• 1-3 years of relevant experience is required.

• Enters client data into software daily.
• Advocates for members and collaborates with the care team.
• Performs all related duties as required under ADA.

Works as a liaison to grant support to the vulnerable members of the community, addressing material and health challenges. Serves as a role model and will identify and initiate referrals to social service programs, serve as main point of contact for the member and support the implementation of various care activities.

• Enters client data into software on a daily basis.
• Advocates for members and their families and collaborates with the care team as needed.
