Qualifications

  • High school diploma or equivalent; one year of data entry and customer service experience.
  • One year of work experience using word processing, spreadsheets, and email applications such as Microsoft Office.
  • Must possess a valid US driver’s license without restrictions and current automobile insurance.

Responsibilities

  • Identify clients at risk of overdose and refer to community services.
  • Coordinate with supervisors, SCHD staff, and partners to connect clients with resources.
  • Travel to client locations including hospitals, clinics, and shelters to provide outreach.
  • Provide health education and information to clients and document work in the database.
  • Assist with scheduling and follow-up on referrals to assess needs and progress.
