Job summary

CAMBA is seeking a Community Health Worker to conduct outreach, assess participant needs, and support families in Brooklyn, NY. You’ll help navigate health and social services, coordinate intake, and connect clients with community resources while working closely with a diverse population.

The role requires a High School diploma or GED plus relevant experience; bilingual English with Haitian Kreyol or Spanish is highly preferred. This is a full-time on-site position at CAMBA’s Brooklyn location.

CAMBA’s Robin Hood Foundation Moving Mothers Health Forward (MMHF) Health Program will consist of imbedded and continuous services provided to the communities of our two Brooklyn-based family shelters and the three NYCHA Public Housing Developments served by CAMBA’s Cornerstone Community Centers.

Position: Community Health Worker

Reports To: Community Health Supervisor I

What The Community Health Worker Does

• Conduct outreach to identify high need women and infants from events at community venues, schools, hospitals, clinics, churches, shelters, substance abuse treatment clinics, emergency rooms etc.
• Plan and utilize creative outreach strategy events (mailings, home visits, presentations, workshops, door-to-door flyer distribution) to motivate participants to agree to use UHF program services.
• Plan and facilitate educational presentations to participants, providers and others regarding MMHF and a variety of topics.
• Conduct initial intake and/or assessment of participants and family situations and needs, and periodic re-assessments.
• Assist participants in completing all CAMBA intake applications and consent forms.
• Create and maintain participant files.
• Manage a caseload across the life cycle.
• In collaboration with participants, prepare initial and periodic revisions of service plans, including short-term and long-term goals.
• Assist participants in attaining their goals by identifying and locating community resources for participants and by making referrals to appropriate services both within and outside CAMBA (i.e., physicians and dentists who can assure appropriate care throughout key life stages and support services like WIC and food stamps).
• Work with participants to overcome barriers/goals, assist participants in advocating for themselves and in moving toward self-sufficiency.
• Act as liaison and advocate for participants with referral sources regarding participant’s progress related to their education, healthcare, housing issues, etc.
• Follow-up with participants for a period of time after successful completion of their primary goals to assure their stability.
• Escort participants to appointments (educational, medical, social service, etc.) and act as translator if necessary.
• Assist participants in completing applications (i.e., assist with gathering documentation) for benefits and entitlements (i.e., health insurance), process applications on participants' behalf and follow-up to ensure enrollment.

Minimum Education/Experience Required

• High School diploma or GED and 4 years of applicable experience and/or equivalent experience.

• An ability to establish trusting relationships and acceptance of individual differences
• Experience and humility to work with culturally diverse families
• Ability and willingness to be available for occasional outreach activities on weekends or evenings.
• Bi-lingual Haitian Kreyol and English or Bi-lingual Spanish and English. (Highly Preferred)

Compensation

$42,426 annually

When salary ranges are listed, the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and candidate. This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization.
