Job summary

Westside Family Healthcare Inc. is seeking a Community Health Worker for its Mobile Health Unit to serve Delaware communities, focusing on migratory and seasonal agricultural workers during harvest season, traveling to farms and housing in Kent and Sussex Counties.

Outside harvest, the CHW engages in MSAW projects and community outreach to connect workers with Westside services, social resources, and health education; this role supports equal access to quality healthcare and health equity.

Qualifications

  • High school diploma or GED is required.
  • Fluency in written and spoken English and Spanish (or English plus another language spoken by farm workers in Delaware).
  • Six months of experience using computers and Microsoft Office software.
  • Driver's license and clean driving record are required.

Responsibilities

  • Maintain relationships with managers and crew leaders to access migratory workers.
  • Travel weekly to farms to provide social services, health education, and follow-up.
  • Register workers in the electronic medical system and update demographics and insurance information.
  • Document all services via encounters and assist with paperwork as needed.
  • Evaluate financial circumstances, collect payments and reconcile charges per route slips.
  • Use designated assessment tools to assess health and social service needs.
  • Develop and translate culturally sensitive health education materials for outreach events.
  • Attend community meetings and participate in events and fairs.
  • Refer uninsured patients to enrollment services for assistance.
  • Assist with offseason MSAW projects and outreach for non-MSAW populations.

Westside Family Healthcare is a nonprofit organization that provides high quality primary medical care without regard to ability to pay. The Community Health Worker for Mobile Health Services engages in outreach efforts for our mobile health clinic. During the harvesting season, the focus is directed toward the migratory and seasonal agricultural worker (MSAW) populations to connect them with the services of Westside Family Healthcare and its collaborating partners. During the harvesting season, the Community Health Worker travels to farms, labor camps and other housing locations within Kent and Sussex Counties to address the health and social service needs of the migratory and seasonal agricultural workers, act as a liaison between them and social services agencies, and participate in health fairs as a lay health educator.

Outside of the harvesting season, the Community Health Worker works on projects related to the MSAW population and others in the community. This will include travel to community service centers to address health and social service needs in the community to connect them with Westside services.

Responsibility of the Community Health Worker - MSAW Program
  1. Maintain relationships with managers and crew leaders to gain access to migratory and seasonal agricultural workers and update crew.
  2. Travel to farms weekly to provide social service resources, health education, enabling services and follow-up to migratory and seasonal agricultural workers.
  3. Register all migratory and seasonal agricultural workers in the electronic medical system, update current patient demographic and insurance information and document all services via Social Services Encounters. Assist all migratory and seasonal agricultural workers who need help with completing any necessary paperwork.
  4. Evaluate financial circumstances, collect payments and co-payments from patients, reconcile daily charges and payments according to route slips.
  5. Utilize designated assessment tool for assessing the overall health and social service needs of agricultural workers and their family members.
  6. Develop, organize, translate, and produce culturally sensitive and competent health education materials needed for agricultural worker outreach and educational events geared towards the targeted population and participate in health education activities.
  7. Attend community meetings and participate in community events/fairs.
  8. Refer uninsured patients to enrollment services for assistance.
  9. In the offseason, assist with MSAW (Migrant and Seasonal Agricultural Worker) related projects as directed and assist patients with in-office health services.
  10. During offseason, participate in outreach and clinical events of non-MSAW populations as directed by MSAW supervisor and Associate Director.
Requirements
Minimum Objective Qualifications
  1. High school diploma or GED
  2. Two years of related experience, such as community services, the healthcare environment or social services work
  3. Fluency in written and spoken English and Spanish, or English and another language spoken by migratory and seasonal agricultural workers working in Delaware.
  4. Six months of experience using computers and Microsoft Office software
  5. Driver's license and clean driving record
Preferred Objective Qualifications
  1. Associate or bachelor's degree in education, business, psychology or another health-related field
  2. Experience in providing services to seasonal and/or migratory agricultural workers.
