Job description

The community health will be eager to work with, and committed to the welfare of, patients who do not consistently engage in care and/or take their prescribed antiretroviral therapy, placing them at risk for opportunistic infections, hospitalization, death and transmission of HIV to others. The community health worker will assist patients with overcoming obstacles such as housing and food instability, transportation, limited health literacy, linguistic barriers, mental illness and substance use disorders. The community health worker will be proficient at helping with any patient-related documentation. The community health worker must be comfortable working with patients from all walks of life, including LGBTQ patients, immigrants and refugees, and people with substance use disorders. Job Summary Summary
Responsible for working closely with individuals and communities to promote and maintain good health. Responsible for providing education, support, and outreach services to community members to help them achieve their health goals. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors. Works under the direction of a healthcare professional to provide preventive care and health education to members of the community.

Essential Functions
-Conduct community outreach and engage community members to understand their health needs
-Provide individual and group health education and coaching to community members
-Assist community members in accessing and navigating the healthcare system, including providing referrals to appropriate health services
-Provide basic health screenings and assessments to identify health risks and provide appropriate interventions
-Assist in the development of health promotion programs and activities to address community health needs
-Collect data and maintain accurate records of community health outreach activities and interventions
-Attend meetings and trainings related to community health promotion and education
Qualifications Education High School Diploma or Equivalent required or Associate's Degree Social Work preferred or Bachelor's Degree Related Field of Study preferred Can this role accept experience in lieu of a degree? No Licenses and Credentials Experience experience in community outreach, health education, or related field 0-1 year preferred Knowledge, Skills and Abilities - Strong communication and interpersonal skills, with the ability to interact effectively with various populations. - Demonstrated ability to work effectively and provide advocacy for all populations and communities preferred. - Ability to work independently and as part of a team. - Basic computer skills, including Microsoft Office and database management. - Bilingual skills preferred.
