Job summary

ChristianaCare in New Castle County, Delaware, seeks a Community Health Worker to bridge patients with healthcare teams and community resources. You will build trust, assess social needs, and help patients set achievable health goals while coordinating care across settings.

You will travel to patients' homes and community sites, coordinating with clinicians and social services to remove barriers to care. A valid driver’s license and reliable vehicle are required.

Qualifications

  • High school diploma or GED.
  • 1-2 years of experience in community outreach, healthcare support, social services, or a related field.
  • Strong communication, relationship-building, organizational, and problem-solving skills.
  • Ability to work independently and collaboratively with patients, healthcare professionals, and community partners.
  • Bilingual communication skills preferred.

Work Schedule & Location

This is a full-time, community-based position in New Castle County with occasional travel to Kent/Sussex Counties. You'll work with patients in their homes, community locations, and ChristianaCare clinical settings. Flexibility to work occasional evenings and weekends is required.

Hourly Pay Range: $26.03 - $39.05This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
