Community Health Worker/ Health Coach – Temporary

Culture Care Collective is a care navigator and health support platform that leverages health coaches and supportive technology to connect people to health and wellness resources. Our platform connects individuals with unmet social needs to community health workers, social supports, and appropriate health services. By providing software to partnering health professionals, our own coaches and clients, we promote healthy learning and connections, bridging gaps in health services to improve overall health and social capital.

This is a part-time, temporary remote role for a Community Health Worker/Health Coach at Culture Care Collective. The role involves providing care navigation to clients, connecting them to health services, social supports, and resources. Current students with peer support/ coaching experience preferred.

Qualifications

1. Experience in care navigation, health coaching, and connecting individuals to resources required
2. Experience supporting students in a college setting preferred
3. Strong communication and interpersonal skills
4. Knowledge of social determinants of health and community resources
5. Ability to work independently and as part of a team
6. Empathy, compassion, and cultural competence
7. Proficiency in using technology and software for health promotion
8. Comfort working in a fast-paced start-up environment
9. Bilingual or multilingual skills are a plus
10. Relevant certification or training in public health, social work, or a related field preferred
