Job summary

IU Health in Indianapolis is seeking a Community Health Worker to provide outreach and support to pregnant individuals and families across pregnancy, labor, and postpartum stages.

You will bridge gaps between communities and healthcare systems, helping clients access prenatal/postpartum care, insurance navigation, housing, food, and transportation resources.

Qualifications

• High School Diploma or equivalent required.
• CHW certification within 1 year of hire.
• 0-3 years of experience required.
• Requires basic computer skills.
• Requires knowledge of medical, community, and social resources.

Responsibilities

• Provide outreach, education, and ongoing support to pregnant individuals and families throughout pregnancy, labor, and the postpartum period.
• Serve as a trusted advocate and bridge between the community and healthcare, government, and social service systems.
• Assist clients in identifying and overcoming barriers to accessing healthcare and community resources.
• Connect individuals to essential services including prenatal/postpartum healthcare, health insurance navigation, food assistance, housing, transportation, childcare, and community health services.
• Conduct assessments to identify client needs and develop referral plans.
• Provide culturally responsive health education related to prenatal care, maternal health, infant care, and healthy lifestyles.
• Support clients in navigating healthcare systems, scheduling appointments, and understanding care plans.
• Offer counseling, encouragement, emotional support, and follow-up services to clients and families.
• Advocate for clients to ensure access to quality healthcare and supportive services.
• Maintain regular communication to monitor progress and address needs.
• Collaborate with healthcare providers and community organizations to coordinate care.
• Facilitate referrals and ensure connections to community resources.
• Maintain documentation of interactions, referrals, services, and outcomes.
• Participate in community outreach and health promotion activities.
• Promote positive maternal and infant health outcomes.

KEY RESPONSIBILITIES

• Provide culturally responsive health education related to prenatal care, maternal health, infant care, and healthy lifestyles.
• Support clients in navigating healthcare systems, scheduling appointments, and understanding recommended care plans.
• Offer informal counseling, encouragement, emotional support, and follow-up services to clients and families.
• Advocate for clients to ensure access to quality healthcare and supportive services.
• Maintain regular communication with clients to monitor progress and address emerging needs.
• Collaborate with healthcare providers, community organizations, and social service agencies to coordinate care and services.
• Facilitate referrals and ensure successful connections to community resources and support networks
• Maintain accurate documentation of client interactions, referrals, services provided, and outcomes.
• Participate in community outreach events, educational programs, and health promotion activities.
• Promote positive maternal and infant health outcomes through education, advocacy, and resource coordination.
• Support family well-being by encouraging self-sufficiency, healthy behaviors, and engagement with available community resources.

REQUIREMENTS

• High School Diploma or equivalent required.
• Must obtain a Community Health Worker certification within 1 year of hire, in accordance with state and organizational standards. Ongoing continuing education may be required to maintain certification.
• 0-3 years of experience required.
• Requires basic computer skills.
• Requires knowledge of medical, community, and social resources.
