• High School Diploma or GED required (minimum education).

• Requires knowledge of medical, community, and social resources.

Responsibilities

• Strengthens connections between patients, care teams, and social services.
• Engages patients in clinic-based programs, education, coaching, and care coordination.
• Facilitates enrollment into Remote Patient Monitoring (RPM) for conditions like hypertension and maternal health.
• Conducts occasional home visits to assess barriers to care and reinforce self-management.
