Heart of Ohio Family Health Centers in Columbus, OH is seeking an RN Care Coordinator for Remote Patient Monitoring (RPM). The role focuses on chronic disease management, reviewing patient data, education, and coordinating care to improve outcomes.

Responsibilities include enrollment in RPM, outreach, medication adherence, transitions of care, and collaboration with the care team. This is a full-time, onsite position reporting to the Clinical Pharmacy and Allied Health Manager.

Formación

• RN license current and active.
• Bachelor's degree in nursing required.
• 0-3 years clinical care experience; ambulatory or primary care preferred.
• CPR certification is required.
• Valid driver's license and reliable transportation.

Responsabilidades

• Monitor and manage patients in RPM programs, focusing on blood pressure and glucose.
• Review, document patient data; communicate abnormal readings and follow up.
• Enroll eligible patients in RPM; educate on equipment, self-monitoring and adherence.
• Conduct outreach related to chronic disease management and preventive care.
• Support medication access and adherence via prior authorizations and outreach.
• Collaborate with patients, families and care teams to coordinate services.
• Serve as primary contact and educational resource for RPM participants.
• Support transitions of care after hospitalizations and ER visits.
• Participate in quality improvement and population health initiatives.
• Provide coverage for RN duties as needed, including nursing assessments and vaccinations.
• Maintain timely documentation and participate in incident review.
