Job summary

Burrell Behavioral Health in Saint Louis, Missouri, is seeking a Community Behavioral Health Liaison to connect individuals with behavioral health and community-based services. Qualifications

  • Bachelor’s degree in social work, psychology, counseling, or a related human services field preferred.
  • Relevant experience in behavioral health, case management, or community services may be considered in lieu of education.
  • Knowledge of community resources and behavioral health systems preferred.

Responsibilities

  • Serve as a liaison between clients, providers, and community resources to ensure coordinated care.
  • Engage individuals and assess needs related to behavioral health, social services, and community support.
  • Facilitate referrals and connect clients to appropriate services including medical, mental health, housing, and social supports.
  • Collaborate with interdisciplinary teams to develop and support individualized service plans.
  • Advocate on behalf of clients to reduce barriers to care and improve access to services.
  • Build and maintain relationships with community partners and service providers.
  • Monitor client progress and follow up to ensure continuity of care.
  • Maintain accurate and timely documentation in accordance with organizational and regulatory standards.
  • Participate in team meetings, case reviews, and program development activities.
  • Support outreach efforts and community engagement initiatives.
  • Promote a recovery-focused, trauma-informed, and culturally responsive approach to services.
  • Perform other duties as assigned by leadership.
