Job summary

Burrell Behavioral Health in Saint Louis, Missouri, is seeking a Community Behavioral Health Liaison to connect individuals with behavioral health and community-based services. You will advocate for clients, assess needs, and coordinate referrals across medical, housing, and social supports to promote recovery and stability.

In this role you will collaborate with interdisciplinary teams, maintain thorough documentation, and participate in program development.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or a related human services field preferred.
• Relevant experience in behavioral health, case management, or community services may be considered in lieu of education.
• Knowledge of community resources and behavioral health systems preferred.

Responsibilities

• Serve as a liaison between clients, providers, and community resources to ensure coordinated care.
• Engage individuals and assess needs related to behavioral health, social services, and community support.
• Facilitate referrals and connect clients to appropriate services including medical, mental health, housing, and social supports.
• Collaborate with interdisciplinary teams to develop and support individualized service plans.
• Advocate on behalf of clients to reduce barriers to care and improve access to services.
• Build and maintain relationships with community partners and service providers.
• Monitor client progress and follow up to ensure continuity of care.
• Maintain accurate and timely documentation in accordance with organizational and regulatory standards.
• Participate in team meetings, case reviews, and program development activities.
• Support outreach efforts and community engagement initiatives.
• Promote a recovery-focused, trauma-informed, and culturally responsive approach to services.
• Perform other duties as assigned by leadership.

Education
