Child & Family Services of Newport County in Rhode Island is seeking a qualified in-home support professional to assist staff and families in a home-based setting. You’ll conduct community outreach, help families access resources, document progress in the EHR, and participate in crisis support rotations, with a hybrid schedule and 20% remote work.
The role requires travel statewide, a valid driver's license, and collaboration with Clinical Care Coordinators and Clinicians on the FSP team, plus
You will provide support to staff and families in a home-based setting, supporting families who have members coping with physical, emotional, socioeconomic, and/or social difficulties. The role includes community outreach activities, helping families practice skills learned during treatment, connecting families with resources, and assisting staff with administrative tasks. You will work closely with the Clinical Care Coordinators and Clinicians on the FSP team.
Hybrid Role, 20% Remote Work.

This position requires time sitting, standing, walking, driving, carrying and lifting up to 25lbs. This role also requires operating desktop, laptop and/or cell phone, and communicating with colleagues.
