Job summary

Compass Health Network is looking for a Community Health Worker to bridge individuals with essential healthcare, social services, and community resources. You will educate, advocate, and provide ongoing support to help people navigate complex systems and improve their health outcomes in our communities.

This role emphasizes health equity, relationship-building, and empowering others, especially in underserved rural communities, with a Monday–Friday 8:00am–5:00pm schedule.
