Job summary
AIDS Healthcare Foundation is seeking a Community Health Worker to provide preventive health services to prevent disease and promote health equity. You will work with members in homes, communities, or clinic settings, supporting adherence to care and connecting them with community resources.
The CHW will address social determinants of health, identify barriers to care, and coordinate with a close-knit team to improve health outcomes. Travel to client sites is expected about 25%.
Qualifications
- Managed Care experience preferred.
- Community Health Worker Certificate Preferred, 2 years to obtain once hired if cert is not held.
- Must have reliable transportation for 25% travel to meet with clients.

A Community Health Worker (CHW) contributes to AHF by blending the mission with care coordination. The CHW provides preventive health services to prevent disease, disability, and other health conditions or their progression; to prolong life; and promote physical and mental health.
A CHW enhances the reputation of AHF by contributing to improved member health outcomes and advancing health equity, ultimately supporting healthier, and happier lives. AHF has a collaborative organizational structure where staff are accountable to multiple leaders.
The CHW will work as a part of a close-knit team to address issues that include, but are not limited to, the control and prevention of chronic conditions or infectious diseases; mental health conditions and substance use disorders; need for preventive services, perinatal health conditions; sexual and reproductive health; environmental and climate-sensitive health issues; oral health; aging; injury; domestic violence; and violence prevention. The CHW proactively assists members by identifying barriers to care and connects them with community resources that support whole-person health. With strong knowledge of available services, the CHW works to address members’ physical, emotional, and social needs. The CHW assists with behavioral health referrals and assists with scheduling appointments. Must have reliable transportation for 25% travel to meet with clients where they sit.
