Program Overview
The Office of Diversion and Re-Entry (ODR) Program serves former criminal justice-involved individuals by providing reentry support services with the goal of reducing recidivism and improving the health outcomes of justice involved populations who have underlying health needs. The holistic and client-centric program assists clients with transitioning out of the justice system with reintegrating into their communities. This population can include individuals with a high emergency room or hospital admission, those who have or are currently experiencing homelessness, those with mental illness and/or substance abuse disorders, or those who may be pregnant.
The Community-Based Intensive Case Management Services Team is responsible for providing resources and supports to improve overall health, social, and economic outcomes of persons served by the program. The CHW will provide intensive case management services to persons who are reintegrating into their communities. The CHW will collaborate directly with the Executive Director, Licensed Clinical Social Worker and staff to ensure that client needs are met with compassion and zeal, through the provision of exceptional case management.
