Job summary

Hamilton Health Center in Harrisburg, PA is seeking a Community Health Worker to identify low-income individuals and assist them in navigating the health care system, including obtaining health benefits and connecting to primary care, dental, vision, and mental health services.

The role requires extensive field work across the service area, with some evenings and weekends necessary to reach patients where they are and provide follow-up support.

Qualifications

• High School diploma or equivalent required.
• Completion of a Community Health Worker formal training program preferred.
• Minimum of one year of experience in community-based care coordination/care management preferred.
• Knowledge of medical terminology.
• Bilingual speaking preferred.
• Reliable transportation to travel throughout the service area.

Responsibilities

• Establishes and maintains trusting working relationships with patients and their families.
• Identifies uninsured patients and connects them to benefit coordinators or other insurance enrollment specialists.
• Conducts intake interviews with patients and refers patients to appropriate social programs.
• Follows up with patients via phone calls and visits to other settings where patients can be found.
• Assists patients in establishing a relationship with a designated primary care provider (PCP), dental, vision, mental health, and supportive service partners.
• Distributes culturally appropriate health information.
• Assists patients with setting and attending appointments.
• Assists patients with connecting to transportation resources. Transporting patients is strictly prohibited.
• Assists patients with navigating systems to obtain resources and/or connect with services, internally and externally.

Job Summary:
The Community Health Worker is responsible for identifying low-income individuals and families and assisting them with navigating the health care system, including arranging for assistance with obtaining health benefits, connecting to a primary care provider or free medical service, and/or accessing needed mental health, dental, and vision care. The position will educate individuals on the appropriate utilization of the health care system to reduce emergency room visits.

This position requires extensive field work, including regular travel throughout the Hamilton service area and the ability to work some evenings and weekends.

Essential Duties and Responsibilities:

The Community Health Worker performs a wide range of duties, including, but not limited to the following:

• Identifies and connects uninsured patients to benefit coordinators or other insurance enrollment specialists.
• Conducts intake interviews with patients and refers patients to appropriate social programs.

• Provides continuous follow-up with patients from initial contact through closure.
• Assists patients in establishing a relationship with a designated primary care provider (PCP), dental, vision, mental health, and supportive service partners.
• Distributes culturally appropriate health information.
• Assists patients with setting and attending appointments.
• Assists patients with connecting to transportation resources. Transporting patients is strictly prohibited.
• Assists patients with navigating systems to obtain resources and/or connect with services, internally and externally.

Minimum Education/Certifications: High School diploma or equivalent required. Completion of a Community Health Worker formal training program preferred.

Minimum Work Experience: Minimum of one year of experience working in a community-based setting performing care coordination/care management in a medical setting preferred.

Knowledge of medical terminology and bilingual speaking preferred.

Other Requirements: Reliable transportation to travel throughout the service area. Ability to work some evenings and weekends.
