Summary
As a Community Health Worker, you will utilize your valuable lived experiences to promote an understanding of the member’s point of view and preference. You will help address chronic conditions, preventive health care needs, and health-related social needs. Your role will work alongside Lead Care Managers (LCM) and paraprofessionals to promote client engagement, provide advocacy and case management support and triage clients to levels of care and community resources as needed.
