Connecticut Children’s Medical Center is seeking a dedicated professional to assist clients in navigating community resources and coordinating care. The role supports linking clients with services and participates in multidisciplinary planning to address medical and psychosocial needs.

Minimum Education and Experience Required:
Education Required: High school diploma or GED.
Experience Required: 1 year experience working in one of the following:Medical case management, community health experience, community organizing/mobilization, faith based community programs, or worked with or experienced children/youth/individuals impacted by social determinants of health or chronic illness/substance/recovery programs.
Experience Preferred:
Living and/or working in an urban setting.
Ability to work with diverse client population
Ability to read and write; bilingual English/Spanish preferred
Knowledge, Skills, and Abilities:
Knowledge of: Community assistance resources
Skilled at: Oral and written communications; interpersonal communications
Ability to: Work with diverse client population, read and write (English/Spanish if required), Flexible schedule: work evenings/weekends as needed
