Job summary

Connecticut Children’s in Connecticut seeks a community health worker to link clients with support services and coordinate with medical staff to address medical and psychosocial needs. You will participate in case conferences, monitor progress, and perform data entry for evaluation.

You will provide education to residents, work with a multidisciplinary team, and help plan and implement care plans for diverse pediatric populations. Flexibility for evenings/weekends may be required.

Qualifications

  • High school diploma or GED required.
  • At least 1 year experience in medical case management or related community health work.
  • Experience in urban settings and with diverse populations preferred.
  • Bilingual English/Spanish preferred.

Job description

Responsibilities
  • Perform referrals and assists clients in linking with community support systems and agencies.
  • Participate in administrative and clinical case conferencing for clients/patients with medical staff/medical case managers in order to address client’s/patients medical and psychosocial needs.
  • Performs data entry tasks for evaluation purposes.
  • Monitor client progress within the health and community support systems.
  • Provides education and training to residents/individuals in their respective community, works as part of the multidisciplinary team to prepare and implement plans, (example for HIV Pediatric and Youth program, Recovery Support Services; and general health awareness. Completes all required documentation.
  • Participate in (in person/Virtual) community and planning meetings such as the Ryan White Planning Council and CT HIV Prevention Council, OCCH community meetings.
  • Other duties as assigned.
Qualifications

Minimum Education and Experience Required:

  • Education Required: High school diploma or GED.
  • Experience Required: 1 year experience working in one of the following: Medical case management, community health experience, community organizing/mobilization, faith based community programs, or worked with or experienced children/youth/individuals impacted by social determinants of health or chronic illness/substance/recovery programs.

Experience Preferred:

  • Living and/or working in an urban setting.
  • Ability to work with diverse client population
  • Ability to read and write; bilingual English/Spanish preferred

Knowledge, Skills, and Abilities:

  • Knowledge of: Community assistance resources
  • Skilled at: Oral and written communications; interpersonal communications
  • Ability to: Work with diverse client population, read and write (English/Spanish if required), Flexible schedule: work evenings/weekends as needed
