Job summary

Family Health Council of Central PA in Camp Hill is seeking a Community Health Worker to support low-income clients with housing and social services. This full-time hybrid role allows some work-from-home days.

The CHW will coordinate with housing, healthcare and social service providers, maintain strong communication, manage time effectively, and travel within the region using a PA driver's license. A High School Diploma or GED is required, and CHW certification is required with training

We are a private, not-for-profit organization that coordinates public health grants for a 26-county region in central PA, seeking a Community Health Worker (CHW). This is a full-time hybrid position located within our Camp Hill location, balanced with opportunities for work-from-home days.

The Community Health Worker (CHW) will provide housing support services and other case management duties as assigned to clients with lower incomes in their communities. CHW will work collaboratively with all applicable internal and external stakeholders' staff and partners to identify needs and connect clients to housing, healthcare and social services.

A High School Diploma or GED (General Education Diploma) is required. Social Services background is plus. CHW Certification will be required; training will be provided upon hire. The successful candidate will also possess strong verbal and written communication skills, time management, and prioritization; computer literacy including Microsoft 365; and the ability to work with cross-disciplinary teams of diverse individuals, including those with lived experience. Reliable transportation and a valid PA driver's license are required for travel within the specified area.

Child Protection Clearances (formerly "Act 153 Clearances")

The following clearances and background checks are required upon employment and as a condition of continued employment:

• PA State Police Criminal Record Check (PATCH)
• FBI Criminal Record Check, from fingerprinting
• PA Child Abuse History Clearance (PACA)
