Job summary

Erie Family Health Centers in Chicago is seeking a full-time Community Health Registered Nurse to provide essential clinical support to patient care activities using nursing assessment and procedural skills. Qualifications

• Current Illinois RN licensure is required.
• Current BLS for Healthcare Provider CPR certification is required.
• One to two years’ experience in community health nursing or ambulatory care preferred.

Responsibilities

• Collaborates with providers in the delivery of patient care.
• Performs triage assessment process of walk-in patients and telephone calls.
• Delivers health education related to medication, diet, treatment of diagnosis, health promotion, prevention and exit health teaching.
• Administers immunizations, applicable contraception refills, and medications as ordered by provider.
• Performs other duties as assigned.
• The work schedule for this role is aligned with Erie's hours of operations.
