Overview

Under the direction of the Community and Social Services Director, the Community Health Representative performs work within the requirements of the Diabetes Grant and Prop 63 for Native Americans, the ICWA program, and the Title V Self-Governance Compact to fulfill community health objectives in those grants. Participates as member of the Community Services Department.

Responsibilities
  • Provides health education, home visits and testing, transportation of patients for appointments, and any medical recommendations at the direction of the Medical Department.
  • Provides home services to ensure the health and safety of Tribal patients at the request of the Community Service Manager and/or Medical Department.
  • Informs the Community Service Manager of findings from home visits when appropriate.
  • Provides transportation services to eligible Native patients according to the standards established in SYTHC transportation policies and procedures.
  • Provides administrative support to the Director of Community and Social Services, including in the programmatic areas of ICWA, food delivery, home visitation, and elder programming.
  • Works to ensure patient and family satisfaction.
  • Establishes a rapport with clients and delivers care in a professional manner.
  • Assists in providing Injury Prevention Programs.
  • Prepared calendar for Community Service Outreach activities.
  • Prepares information for monthly Table 1 reports to Indian Health Services.

  • Performs other duties as assigned.
Qualifications
  • High School Diploma or GED Certificate.
  • Bachelor’s Degree in a Healthcare related field or equivalent work experience.
  • Possession of CPR Certification and First Aid Certification within 60 days of hire.
  • Certified Medical Assistant is preferred.
  • Three years prior experience in a healthcare setting.
  • Basic computer proficiency utilizing Microsoft applications, email, and internet.
  • Basic proficiency in NextGen software is preferred.
  • Native American hiring preference applies.
