iCIMS, Inc. is seeking a Community Health Representative to support community health objectives under various grants. Located in Santa Ynez, you will provide health education and home services, establish rapport with clients, and inform community service management.
The ideal candidate will have a High School Diploma or GED, a Bachelor’s Degree in a healthcare field, and three years of healthcare experience. CPR and First aid certifications are necessary within 60 days of hire, with preference for Certified Medical Assistants.
Under the direction of the Community and Social Services Director, the Community Health Representative performs work within the requirements of the Diabetes Grant and Prop 63 for Native Americans, the ICWA program, and the Title V Self-Governance Compact to fulfill community health objectives in those grants. Participates as member of the Community Services Department.
