Responsibilities

• Assists with planning and implementation of community health program(s).
• Engages with local organizations and care providers to promote program participation.
• Coordinates operations, communication, logistics, and documentation of community health initiatives.
• Collects, tracks, and reports program data to leadership for improvement and compliance.
• Contributes to policies and programs to improve community health delivery.

Education

High School Diploma or equivalent

Associate’s or Bachelor’s degree in relevant field
