Bon Secours Mercy Health is seeking a Community Health Program Coordinator at Lourdes Hospital to support the implementation of community health programs. You will work with partners across the organization, system, and community to ensure smooth program operation and evidence-driven outcomes.

The role emphasizes collaboration with government agencies, faith-based institutions, and non-profit organizations, with opportunities to contribute to policy input and program improvements for community

Qualifications

• High School Diploma or equivalent is required.
• Associate’s or Bachelor’s degree in a relevant field is preferred.
• 1 year related experience is required.
• 3 years related experience is preferred.

Responsibilities

• Assists with planning and implementation of community health programs.
• Participates in outreach efforts with local organizations, community members, and care providers.
• Coordinates operations and support related to planning, communications, logistics and documentation.
• May collect, track, and report program data to leadership for improvement and compliance.
• Provides input into policies and programs that improve community health.

Community Health Program Coordinator – Lourdes Hospital

Job Summary

The Community Health Program Coordinator supports the implementation of health programs within a community. This role works closely with partners across the organization, system, and/or community including government agencies, faith‑based institutions, and non‑profit organizations to ensure the community health programs run smoothly.

Essential Functions

• Assists with planning and implementation of community health program(s) in support of advancing Community Health goals.


• Participates in outreach efforts, which may include engaging with local organizations, community members, and care providers to facilitate collaboration and program delivery by promoting program participation and raising awareness of health services.


• Coordinates operations and support related to planning, communication/promotions, logistics, and documentation of community health initiatives to reach diverse populations.


• May collect, track, and report program data to leadership to support continuous improvement and compliance.


• Provides input into policies and programs that improve the health of the community served including new interventions and improved delivery of health services.
