Community Health Outreach Specialist- Long Island, NY

Job Description

The Community Health Outreach Specialist is responsible for engaging with hard-to-reach, high-acuity patients and helping them access Monogram Health’s medical professionals.

Roles and Responsibilities
  • Conduct initial and ongoing outreach to high-risk patients, educating them about the Monogram Health program and ensuring they access its benefits.
  • Communicate the benefits of the program, address objections, and promote enrollment.
  • Extensive daily driving to visit community members in their homes and connect them with care providers.
  • Provide warm handoffs to Monogram Health clinicians and support staff, ensuring successful scheduling of initial enrollment appointments.
Position Requirements
  • Location: Long Island, NY
  • Heavy travel required, primarily driving to patient homes.
  • Valid driver’s license.
  • At least 2 years’ experience in healthcare, sales, outreach, engagement, or enrollment campaigns.
  • Proficiency with Salesforce and Microsoft Office.
  • Bachelor’s Degree preferred but not required.
  • Minimum 2 years of B2B sales experience.
