Job summary

Iris Family Support Center is seeking a Community Health and Outreach Coordinator in Fort Wayne, IN to connect families with needed health and social services. You will help reduce barriers to care, support mental and medical health, and work with the Prevention Education Team to educate students and adults on various topics.

Qualifications

• Bachelor’s degree in human services, Social Work, Education or related field.
• One year of full-time experience working with families/children in human services is preferred.
• Reliable daily transportation and valid driver’s license with insurance.

Responsibilities

• Assess needs of families to refer to appropriate services in the community.
• Conduct outreach to engage families into services (face-to-face, drop-bys, calls/texts).
• Complete case notes in a detailed, finished format.
• Maintain data including demographics, referral sources, and medical visits.
• Collaborate with partners inside the organization and in the community.
• Establish trusting relationships with parents using independent judgment.
• Assist families with medical and follow up appointments as needed.
• Travel within service area and coordinate with community resources.
• Educate the community on child abuse prevention, safety topics, and related areas.
• Attend 12 hours of in-service training per year.
