Job summary

Berkshire Health Systems is seeking a home health nurse who will provide, teach, supervise, and document nursing care at patients' homes to promote health, prevent illness, and support rehabilitation. You will collaborate with physicians, social workers, and community teams to coordinate care and evaluate patient responses to interventions.

The role requires MA RN licensure, a BSN recommendation, NLN-accredited program graduate, and willingness to be flexible with on-call scheduling and driving

Job description

This position requires working every third weekend and three holidays/year.

DEFINITION / PRIMARY FUNCTION

This role provides, teaches, supervises and documents nursing care of the sick and injured at home to promote, maintain or restore health, prevent illness, and effect rehabilitation. The role collaborates with other healthcare providers, professionals and community representatives to coordinate care at home, evaluating the individual and family’s response to interventions.

POSITION QUALIFICATIONS (Minimum qualifications are required unless stated otherwise.)

Experience

• One-year acute nursing care preferred, hospital based preferred.
• Experience in home care setting strongly preferred.
• Experience with self-scheduling of caseload preferred.

Education and Training

• Bachelor of Science in Nursing recommended.
• Graduate of an N.L.N. accredited nursing program.

License, Certification & Registration

• RN License - Current license as a professional nurse within MA
• Active and valid driver's license.
• CPR required every two years.

• Demonstrated ability to perform independently as a clinician with minimal supervision or assistance.
