Job summary

Neighborhood Outreach Access to Health (NOAH) at Midtown Health Center in Phoenix, AZ seeks a qualified Nurse Practitioner to provide and manage direct medical evaluation, diagnosis, and treatment in line with evidence-based guidelines.

The NP will prescribe medications, update records, and coordinate care with nursing staff, referring to specialists as needed. Bilingual English/Spanish preferred; DEA and BLS certifications required.

Qualifications

• Master's degree in nursing from NLN/CCNE.
• National NP certification from a recognized agency.
• Arizona NP license with prescriptive authority.
• DEA certification and BLS certification are required.
• X-waivered is preferred.
• 1+ years of NP experience preferred.

Responsibilities

• Provides direct patient care including examinations, evaluations, and treatment.
• Prescribes medications and treatment regimens as appropriate.
• Reviews and updates medical records in compliance with standards.
• Refers patients to specialists when needed.
• Communicates with patients and families to engage them in care decisions.
• Provides counseling on wellness, prevention, and health maintenance.
• Coordinates patient care activities with nursing and support staff.
• Adheres to PPE standards and safety protocols.
• Attends meetings and community outreach activities.
