This position provides public health services, community outreach, and health education to support the well-being of individuals and families. As a Community Health Nurse, you will perform the following duties:

• Health Assessment: Analyze county data to identify public health needs and guide education and clinical activities
• Community Outreach: Link program resources with community partners through meetings, clinics, and coordinated activities
• Public Health Initiatives: Develop programs based on local data, nursing guidelines, and identified community priorities
• Program Monitoring: Track health programs and use information systems to support disease prevention and improve outcomes
• Education Delivery: Provide health education on key public health topics to inform and engage community members

Work Schedule and Additional Information

• Full-time employment, 37.5 hours per week
• Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
• Work infrequently requires statewide travel and irregular or flexible work hours, including overnights, weekends, and holidays.
• Telework: You will not have the option to telework in this position.

QUALIFICATIONS

Special Requirements

• This position requires active authorization to practice as a Registered Nurse in Pennsylvania.
• If you possess an active temporary practice permit, you must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.
