Job summary

Penn State Health is seeking a Community Health Worker to engage, support, advocate and educate community members on health and wellness, connecting individuals with resources related to Social Determinants of Health.

As a frontline public health worker, you will act as a liaison between health and social services to facilitate access and improve health outcomes through outreach, informal counseling, social support and advocacy. Position is grant funded.

Qualifications

• High School diploma or equivalent.
• Minimum one year experience of community outreach.
• Valid PA driver's license.
