Job summary

Your Health Organization in Georgia is seeking a dedicated Community Health Worker to support patients across Athens, Hartwell, Elberton and nearby areas. You will visit patients at home and in assisted living facilities, coordinate with care teams, and help patients access services and medications.

Ideal candidates will have relevant certification or licensure, experience with home health, and comfort using EMR, RPM, and telemedicine tools.

Qualifications

Certified Medical Assistant, Paramedic, or Licensed Nurse preferred.
Phlebotomy experience preferred; willing to train if needed.
Experience in public health or home health preferred.
Experience with DME delivery or logistics operations preferred.
Proficient with remote patient monitoring, telemedicine, and EMR systems.
Strong organizational and time-management skills; multitask effectively.
Ability to read and communicate effectively; strong written and verbal skills.
Basic computer knowledge.
Able to work independently with minimal supervision.
Good interpersonal and communication skills under all conditions.
Must be able to work flexible hours and travel between offices, facilities, etc.
Must have a valid driver’s license with insured vehicle in good standing.

Responsibilities

Work closely with care team to ensure coordinated patient care and compliance with care plan.
Communicate via phone, email, Teams, Athena, in-person, etc.
Coordinate visits with PTs, OTs and Providers in ALFs and patients' homes.
Deliver medications to patients' homes, review instructions, and perform medication reconciliation.
Coordinate telehealth visits between patients and pharmacists, specialists, therapists, and providers.
Set up training and support for Remote Patient Monitoring equipment and troubleshoot when necessary.
Deliver durable medical equipment (DME) to homes and ALFs, provide training on use.
Perform routine venipuncture for laboratory testing and ensure proper specimen handling.
Collect blood specimens during home or facility visits.
