Placements24 is seeking a compassionate and resourceful Community Health Navigator to join our client’s team in Welkom, South Africa. The role focuses on helping individuals overcome barriers to essential healthcare services and connecting them with providers and community resources.

You will guide clients through the healthcare system, assist with scheduling, support chronic disease management, and conduct outreach to raise awareness of local health resources.

Qualifications

• Experience in community health, social work, or a related healthcare field.
• Strong understanding of local healthcare landscape in Welkom.
• Excellent communication and motivational interviewing skills.

Responsibilities

• Provide personalized guidance to clients navigating the healthcare system.
• Assist clients in scheduling medical appointments and understanding treatment plans.
• Connect clients with health and social services, including mental health support and chronic disease programs.
• Conduct outreach to raise awareness about health resources in the community.
