Job summary

HRCAP Inc. seeks a Community Health Outreach Navigator to connect individuals and families with healthcare services, public benefits, food assistance, and community resources to improve quality of life and self-sufficiency.

You will collaborate with healthcare providers, food banks, government agencies, and community partners across Hampton Roads to reduce barriers for vulnerable populations. Requires strong communication and data-tracking abilities.

Qualifications

• Obtain membership with Virginia Community Health Worker Association within six months of employment.
• Obtain Community Health Worker certification within 90 working days of employment.
• Bachelor's degree in human services, social services, sociology, or psychology; experience with TANF and VIEW eligibility.

Responsibilities

• Assist individuals and families in identifying and accessing healthcare, behavioral health, nutrition, housing, employment, transportation, and other resources.
• Conduct comprehensive needs assessments to identify barriers impacting health and self-sufficiency.
• Provide resource referrals and follow-up to ensure connections to services.
