Job summary

Qualifications

• High School Education or equivalent.
• 1 year experience in a primary care setting or related field.
• CHW Certification or ability to obtain within 1 year.
• Understanding of health care delivery including insurance eligibility and enrollment.
• Experience with case management for complex patients with barriers to care.
• Strong interpersonal and communication abilities.
• Public speaking comfort and skill.
• Knowledge of community resources and ability to connect patients with services.
• Proficient with computers and software including EHR.
• Available for weekend/evening duties and multiple non-traditional work sites.
• Ability to work independently with good judgement.
• Well-organized, flexible, and able to manage multiple tasks.
