CommonSpirit Health, based in Minnesota, seeks a Community Health Advocate to bridge clinical teams and the community, helping patients navigate care, overcome barriers, and promote healthy lifestyles.

You will connect patients with community resources, provide informal counseling, and advocate for health equity through outreach, coordinating with local agencies and documenting activities in the EMR.

Formación

• High School Diploma required.
• Associates Degree preferred.
• Experience in community health or social services is a plus.

Responsabilidades

• Establish partnerships with patients, families, and healthcare teams to provide ongoing advocacy.
• Perform case management and follow-up to ensure continuity of care.
• Assess social needs and assist patients in goal setting using motivational interviewing.
• Coordinate community resources and referrals to external agencies.
• Assist with paperwork and document in the EMR.
• Participate in staff meetings and interdisciplinary discussions.
