Job summary

CVS Health Corporation is seeking a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to deliver evidence-based patient care in the community setting in Minnesota. Qualifications

• Active and in good standing LVN or LPN license.
• NCLEX-PN completion (National Council Licensure Examination for Practical Nurses).
• Knowledge of medical terminology and clinical documentation standards.
• Experience with phlebotomy, vaccine administration, and point-of-care testing.
• Excellent communication and organizational skills.
• Experience in a clinical setting with diverse cultural dynamics.
• Proven ability to manage sensitive information ethically and responsibly.
• Strong problem-solving, critical thinking, and decision-making skills.
• Proficiency with electronic medical record (EMR) systems; Epic experience preferred.

Responsibilities

• Administer oral, injectable, intravenous, and topical medications.
• Evaluate complex patient conditions, monitor pain levels, and document treatment responses.
• Apply advanced vital sign monitoring to identify abnormalities.
• Perform routine assessments to monitor health and well-being.
• Manage patient access to healthcare and connect with community resources.
• Conduct medication reviews to identify side effects and drug interactions.
• Develop and administer mental health assessments to screen for depression and evaluate mood and cognition.
• Document patient medical history using electronic records.
• Interact with patients, families and healthcare professionals.
