Job Overview

The Community Health Evaluation and Reporting Specialist is responsible for collecting, analyzing, and reporting data on community health initiatives, programs, and impact metrics. This role ensures that all tracking and reporting meet relevant grant, contract, regulatory, and compliance requirements. The specialist will lead Fairview’s community health needs assessments (CHNAs), implementation strategies, and action plans while helping to shape evaluation frameworks and reporting processes that drive informed decision-making.

The Community Health Evaluation and Reporting Specialist also plays a key role in highlighting the root causes of health disparities and ensuring that diverse communities’ experiences are accurately represented in evaluations and reports. The ideal candidate will bring strong analytical, writing, and stakeholder engagement skills, as well as experience working with equity-focused evaluation methodologies.

This position is grant funded for 12 months..

Writing & Content Development

· Manage the end-to-end reporting process, ensuring timely submission and alignment with goals.

· Oversee multiple reporting projects simultaneously, coordinating with internal teams to meet deadlines and quality expectations.

· Develop, draft, and submit high-quality reports including the Community Health Needs Assessments in compliance with regulatory, grant, contract, and program standards.

· Develop, draft, and submit high-quality reports and presentations for essential documents, including annual tax narratives, program one-pagers, impact summaries, evaluation reports, and strategic recommendations.

· Collaborate with key internal stakeholders including communications team to refine reports and effectively disseminate findings to stakeholders.

Regulatory Compliance & Reporting

· Lead the system-wide community health assessment cycle for all Fairview hospitals, including project management, tool development, implementation, analysis, and reporting.

· Identify, recommend, and monitor community-level indicators that support health equity strategies and impact goals.

· Ensure all reporting processes align with IRS regulatory, accreditation, and assessment-specific compliance requirements.

· Support grant or contract funded initiatives by tracking key metrics, drafting narratives, and providing data-driven insights.

Program Evaluation

· Project manage end-to-end program evaluation ensuring timely submission and alignment with goals including coordinating with internal teams to meet deadlines and quality expectations including literature reviews, assessments, tool design, and programmatic insights.

· Develop and implement evaluation frameworks to measure the effectiveness of community health programs.

· Manage an annual reporting calendar to ensure compliance with program, grant, contract, and regulatory requirements.

· Oversee the evaluation lifecycle of community health programs, including theory of change development, impact measurement, and reporting.

· Conduct qualitative and quantitative analysis centering on historically marginalized communities, health disparities, and social determinants of health.

· Lead the development of standardized measurement processes for community impact reports, assessment reports, and dashboards.

· Establish criteria and processes for program activity reporting and outcome evaluation.

Stakeholder Engagement & Collaboration

· Develop and provide educational opportunities for staff on equity-informed evaluation methodologies, including logic models, survey design, and performance tracking.

· Partner with internal teams, public health agencies, academic institutions, and community organizations to strengthen evaluation efforts.

· Collaborate with researchers, epidemiologists, and analysts to align evaluation strategies with community needs.

· Develop and present trainings, dashboards, and reports to effectively communicate key findings.

Education

· Bachelor’s degree in public health, healthcare, philosophy, business, or related field or combination of equivalent experience and education

Experience

· 3+ years experience in program evaluation and project management

· 2+ years of experience writing reports, grant proposals or impact narratives for diverse audiences

· Experience translating complex data into accessible presentations and reports

· Experience with RedCap.

· Strong communication and organizational skills, with attention to detail.

Preferred

Education

· Bachelor’s degree in public health, healthcare, philosophy, business, or related field

Experience

· 4+ years of experience in program evaluation and project management

· 3+ years of experience developing evaluation plans, logic model, and program impact assessments.

· 1+ years of experience applying an equity-focused approach to evaluative design and/or analysis

· 2+ years of experience writing reports, grant proposals or impact narratives for diverse audiences
