Qualifications

• Bachelor's degree in public health, education, health administration, health education, or related field.
• Previous experience with community stakeholders and diverse communities.
• Evenings and weekends availability required.

Responsibilities

• Collaborate with hospital leaders, departments, and community partners to address health disparities.
• Develop goals, strategies, action plans, and performance measures.
• Serve as hospital liaison to community health coalitions and initiatives.
• Build partnerships to advance CHNA/CHIP/SIP priorities.
• Support CHNA process, program evaluation, and data reporting.
