RWJBarnabas Health is seeking a Community Health Worker in Jersey City, NJ for evening shifts. You will connect patients to resources, educate on health and social needs, and navigate care with multidisciplinary teams.

A high school diploma and a valid driver’s license are required; Spanish is a plus. The role involves in-home and community visits, documentation, and collaboration with clinics and hospitals to reduce barriers to care and promote health equity.

Qualifications

• High school diploma or equivalent is required.
• Reliable transportation and valid driver's license; frequent travel in Hudson and Bergen County.

Responsabilités

• Meet patients in clinics, hospitals, and community settings to assess Social Determinants of Health and identify barriers to care.
• Build trusting relationships and provide education, encouragement, and ongoing support for healthy lifestyles.
• Schedule weekly follow-up calls and home visits to monitor progress and needs.
• Assist patients with housing, food insecurity, transportation, and other social needs.
• Help organize medical records, coordinate appointments, and navigate healthcare services.
• Connect patients with community resources such as food assistance, housing programs, and transportation services.
• Collaborate with care teams to coordinate services and improve continuity of care.
• Travel within the community to meet patients at homes, churches, shelters, or hospitals.
