Job summary

Mass General Brigham is seeking a Community Health Worker to collaborate with individuals and communities to promote and maintain good health. The role provides education, support, and outreach services to help people achieve their health goals and navigate the healthcare system.

Under supervision of a healthcare professional, you will conduct outreach, deliver health education, perform basic screenings, collect data, and participate in program development and trainings to advance community

Qualifications

• High School Diploma or Equivalent required; Social Work or related Bachelor's preferred.
• Experience in community outreach, health education, or related field 0-1 year preferred.
• Bilingual skills preferred.

Responsibilities

• Conduct community outreach and engage community members to understand their health needs.
• Provide individual and group health education and coaching to community members.
• Assist community members in accessing and navigating the healthcare system, including referrals to appropriate health services.
• Provide basic health screenings and assessments to identify health risks and provide appropriate interventions.
• Collect data and maintain accurate records of community health outreach activities and interventions.
• Attend meetings and trainings related to community health promotion and education.
