Job summary

Under supervision, the role supports access to care, provides basic health screenings, and assists in linking individuals to appropriate health services and programs. Hybrid work arrangement with on-site emphasis.

Qualifications

• High School Diploma or equivalent required; Social Work degree preferred.
• Experience in community outreach, health education, or related field preferred.
• Bilingual in Spanish and English required.
• Ability to use basic computer programs and maintain records.

Responsibilities

• Conduct community outreach and engage community members to understand their health needs.
• Provide health education and coaching to community members.
• Assist community members in navigating the healthcare system and referrals to appropriate services.
• Provide basic health screenings and assessments to identify health risks and provide interventions.
• Attend meetings and trainings related to community health promotion and education.
